You must select one of the three options: male, female, or unspecified. Question 4: Date of birth This question asks you to provide your date of birth as it appears on your passport or travel document. You must write your date of birth in the format YYYY-MM-DD, where YYYY is the year, MM is the month, and DD is the day. For example, if you were born on January 15, 1990, you would write 1990-01-15.
